How to Write Notes that Keep Clients Safe in Systems That Don’t

In today’s sociopolitical climate where deportations, state surveillance, and government overreach are increasingly normalized therapists are facing a critical question:

“What do I write in my notes that won’t get my client hurt?”

Many therapists are feeling the rising pressure of documentation under authoritarian conditions. With intensified scrutiny of client records by courts, immigration authorities, insurers, and hostile institutions, the stakes of a single word in your notes have never been higher.

This training offers a clear, compassionate path forward: how to document ethically, minimally, and protectively without sacrificing your values or your clients' dignity.

🔍 You’ll Learn How To:

  • Write legally compliant, trauma-informed clinical notes that reduce harm

  • Avoid common documentation pitfalls that increase risk for clients in marginalized groups

  • Embed treatment goals directly in your notes (without relying on EHR treatment planners)

  • Update outdated and pathologizing language with safer, values-aligned phrasing

  • Write about trauma, substance use and Addiction, immigration, reproductive issues, gender identity, sexual orientation, suicidal parts, and systemic violence with clarity and care

  • Navigate documentation when working with clients impacted by immigration policy, state custody, or carceral systems

  • 3 CEUs for Texas-based clinicians (LCSWs, LPCs, LMFTs)

  • The complete Wholehearted Narrative Note template
